Completing in-person visits to determine eligibility for continued hospice services in coordination with the Hospice Medical Director and nursing team. Support nursing team to address symptom management needs in collaboration with the hospice pharmacist.
Mileage and drive time are paid.
Job Summary
The Certified Nurse Practitioner, in collaboration and consultation with physicians, staff RNs, and other health care professionals, provides holistic, compassionate care to clients and families. The Nurse Practitioner practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. The Nurse Practitioner must have an advanced nursing practice that includes independent nursing functions based on the applicable Scope of Practice, nursing standards of care, and a role in medical management within a collaborative agreement with a physician(s).
Responsibilities include:
- Performing in-depth health risk assessments on all assigned clients.
- Working collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel.
- Identifying overall quality improvement activities through assessing and planning health care individualized to the client.
- Reviewing and interpreting the medical, surgical, and social history of clients.
- Performing appropriate screening procedures for clients.
- Interpreting laboratory and other diagnostic data and recommending appropriate treatments or actions for follow-up.
- Prescribing or performing various treatment modalities including medications, injections, wellness counseling, suturing, and ancillary testing.
- Assisting hospice location employees in maximizing client quality of life.
- Evaluating clients who have acute changes in condition and collaborating with the primary care provider and employees to implement appropriate interventions.
- Collaborating with the interdisciplinary team to create, review, and revise the client's plan of care.
- Providing medical expertise on pain, symptom management, and the evaluation of terminal prognosis to clients, family members, and staff.
Qualifications
Completion of a master’s, postmaster’s, or doctorate from a nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission. Licensing prior to August 1, 1995 in ND; master's degree in nursing is preferred.
Minimum two years clinical experience as a Nurse Practitioner in a geriatric, adult, or family practice setting. Experience with palliative care or hospice preferred.
Current licensure by the applicable state board of nursing for advanced practice. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances.
Current certification by the American Nurses Credentialing Center or an equivalent nationally recognized body. Submit current Scope of Practice that has been approved by the applicable state Board of Nursing for advanced practice.
